What is Premium & Claims Reconciliation?

It is proving that the premium, claims and reserve balances in your policy admin, billing, claims, reinsurance and reserving systems agree with the general ledger at each close — and explaining every difference. Premium & Claims Reconciliation has agents tie each subledger to its ledger account, match the lines, cluster what does not match by cause and draft the fixes for the controller.

The problem

Why the subledgers never tie on the first pass

At every close, written and unearned premium, premiums receivable, paid losses and LAE, case reserves, salvage and subrogation and ceded recoverables come out of the policy admin, billing, claims, reinsurance and reserving systems — and they rarely agree with the general ledger. The subledgers post line by line on effective date; the ledger takes daily summaries plus manual journals.

So a top-side reserve, a duplicated accrual reversal and a new cost code mapped to the wrong account all look the same: tens of thousands of unmatched lines. Controllers rebuild the same spreadsheets every month, sort the lines by hand and find on day four or five that a seven-figure break was three journals. What nobody had time to explain is carried forward, or written off late.

One quarter close, from extracts to a signed tie

Larkspur Mutual Insurance Group, Q3 2026 close, business day 1 of 6. Case loss reserves are out by $2,131,418.62. Here is what happens next, screen by screen, in the working solution.

  1. 0107:40 · day 1

Ten ties on one board before her day starts

    Teodora Lind · Financial controller

The extracts landed at 06:00. By 07:40 six agents have loaded 2,941,207 lines, proved the control totals and tied ten insurance subledger accounts to the September ledger: 4 tie to the cent, 1 is explained, 5 are out of balance — $2,625,521.69 out, gross. The biggest is 2105 Case loss reserves: subledger $412,806,540.12, ledger $414,937,958.74.

    “40,318 unmatched lines of 1,288,412 · each dot ≈ 100 lines”

  2. 02One click

    Forty thousand lines, three causes

    Cause finder

Teodora clusters the break by cause. 38,977 lines are timing — reserve changes entered after the 18:00 claims batch on Sep 30, in the ledger on the Oct 1 batch, paired claim by claim — and net to $0.00. 1,338 are mapping on retired cost code CA-HIRED and also net to $0.00. Three lines are manual journals, and they carry the whole −$2,131,418.62.

No cluster is explained without a source record.

  3. 03Traced at 07:21

    Three journals, three different answers

    Journal tracer

JE-2609-0418 is the Q3 top-side case reserve strengthening for Commercial Auto, $1,750,000.00, approved by the reserve committee on Sep 24 — keep it as a documented reconciling item. JE-2609-0433 reverses the August settlement accrual on the Hollis Freight Lines claim a second time: JE-2609-0391 already did it on Sep 3. JE-2609-0447 booked the Dunmore Logistics subrogation recoverable, $30,581.38, against case reserves instead of 1270.

“The whole −$2,131,418.62 is three manual journals.”

  4. 04The break file

Every step on the tie, with the source beside it

    Ines Calloway · Claims accountant

The break file for 2105 shows the agents' run on this tie from the 06:00 extract check to the drafted entries, and each journal with who posted it, who approved it and its support. The reserve committee minutes of Sep 24 sit next to the top-side; the two journal lines on claim CA-26-004871 sit side by side, same claim, same amount, same accounts.

  5. 05Drafted at 07:33

Adjusting entries with their support, never posted

    Entry drafter

AJE-Q3-07 reverses the duplicate: debit 2105-CA, credit 5105-CA, $412,000.00. AJE-Q3-08 reclasses the Dunmore recoverable from 2105-CA to 1270-CA, $30,581.38. Each entry shows its effect on every tie and its approval chain — drafted by the agent, reviewed by Ines Calloway, approved by the controller. Five entries are waiting in all, $839,581.95.

“Nothing posts until you approve; the CFO co-approves $1M or more.”

  6. 06Approve and keep

Two entries approved, one journal kept

    Teodora Lind · Financial controller

One dialog holds the decision: keep JE-2609-0418, post AJE-Q3-07 and AJE-Q3-08 on the next ledger batch at 09:15. Each entry is under $1,000,000, so no CFO co-approval is needed, and the dialog confirms she is not the preparer. She signs with her password and the meaning of the signature: “Approved as financial controller”.

  7. 07Signed 09:12

The tie closes with one explained item

    Teodora Lind · Financial controller

Case reserves move from −$2,131,418.62 out of balance to −$1,750,000.00 explained — the Q3 top-side, documented with the minutes. Salvage and subrogation now ties. The board reads 5 tied, 2 explained, 3 out of balance.

“Explained. One documented item, −$1,750,000.00, the Q3 top-side. Ready to sign.”

  8. 08Same morning

    The other breaks, already explained

    Marcus Bell · Premium accountant

On the Breaks list every cluster says what it is and what resolves it. 214 Harrowgate Agency Partners lockbox payments arrived without policy numbers and sit in 2290 Unapplied cash (AJE-Q3-10). 61 independent adjuster invoices on new cost code IA-FEE-26 map to 5300 instead of 5210 (AJE-Q3-09). 18 marine quota share lines billed in GBP were revalued at different rates (AJE-Q3-11). Three homeowners cancellations after the 20:00 cut-off clear on the Oct 1 batch.

  9. 09Every close

    Nothing carried silently

    Carry-forward watcher

31 reconciling items were carried into the Q3 close. Two open items are over 60 days and flagged to escalate: the Harrowgate July sweep in unapplied cash, $18,440.00, and cash in transit on the property catastrophe treaty, $96,300.00. Premiums receivable over 90 days — $1,842,116.30 across 312 accounts — is shown as non-admitted on the statutory statement.

The carry-forward watcher never writes an item off.

  10. 10For audit

A pack where every item links to its source

Owen Pratt · Internal audit manager

The audit pack holds the summary, all ten ties, the reconciling items, the adjusting entries with who approved them, and an evidence index of 13 sources — from claims extract CLM-RSV-20260930 to the reserve committee minutes and the GBP/USD rate table. It is prepared for internal audit and the Model Audit Rule file, and exports to PDF.

Built on the engine

6 agents. Each with one job, and hard limits.

Six specialist agents load, tie, match, cluster by cause, trace the journals and draft the entries; the financial controller approves and signs.

Extract checker

Loads the policy, billing, claims, reinsurance and reserving extracts and the trial balance at 06:00 on business day 1, and proves record counts and control totals against each source.

  • Read-only access to every source
  • Stops the run on a control total that does not agree
  • Never changes a source record or a ledger balance
Tie-out matcher

Ties each subledger to its ledger account, then matches lines by claim, policy or treaty, amount and effective date — one-to-one, one-to-many and many-to-one.

  • Never matches outside the tolerance in Settings
  • Shows the rule behind every match
Cause finder

Clusters the unmatched lines of each tie by cause — timing, mapping, manual journal, currency, missing feed — and nets each cluster.

  • No cluster is explained without a source record
  • Shows lines and net for every cluster
Journal tracer

Reads each manual journal in a break — header, preparer, approver and support such as memos, minutes and claim notes — and flags duplicates, wrong accounts and journals without support.

  • Never accepts a journal without its support
  • Flags a journal where preparer and approver are the same person
Entry drafter

Drafts adjusting entries with debit and credit lines, a description and the support for each, shows the effect on every tie and routes them for approval.

  • Cannot post — only a person approves
  • The CFO co-approves entries of $1M or more
Carry-forward watcher

Ages every reconciling item carried from earlier closes, escalates items over 60 days to the owner and controller, and reports premiums receivable over 90 days as non-admitted.

  • Never writes an item off
Financial controller

Approves entries and signs; CFO co-approves $1M or more. The agents propose; a named person decides.

Ask in plain words

Ask about any tie, break or journal

The controller and preparers can ask in plain words — or tell it what to change. Every answer cites the source record.

Why is case reserves out by $2.1M?

Three manual journals — nothing in the 40,318 lines. 38,977 lines are timing, after the 18:00 claims batch, and 1,338 are mapping on retired code CA-HIRED; both net to $0.00. The $2,131,418.62 is the Q3 top-side of $1,750,000.00 approved on Sep 24, a duplicate reversal of $412,000.00, and a $30,581.38 subrogation recoverable booked against the wrong account.

What will still be open after today’s entries?

If you approve the five drafted entries, three items stay, all explained: the $1,750,000.00 Commercial Auto top-side, documented with the minutes; and 3 unearned premium cancellations, $52,300.00, and 9 returned payments, $14,221.12 — both clear on the Oct 1 batch.

Map IA-FEE-26 to 5210 from October

Done as a proposal. Claims accounting policy CAP-7 puts independent adjuster fees in 5210. IA-FEE-26 → 5210 Adjusting and other expense is added to mapping version 15, effective Oct 1, for your approval in Settings. The 61 September invoices are still corrected by AJE-Q3-09.

Show receivables over 90 days

$1,842,116.30 across 312 accounts is over 90 days past due and is non-admitted for the statutory statement — down from $2,014,880.12 at June 30.

Questions

What controllers and close teams ask us.

What is premium and claims reconciliation?

It is proving that the premium, claims and reserve balances in your policy admin, billing, claims, reinsurance and reserving systems agree with the general ledger at each close — and explaining every difference. Premium & Claims Reconciliation has agents tie each subledger to its ledger account, match the lines, cluster what does not match by cause and draft the fixes for the controller.

How does it explain a case reserve break?

The cause finder groups the unmatched lines into timing, mapping, manual journal and currency clusters and nets each one. In the working solution a −$2,131,418.62 break across 40,318 lines comes down to three manual journals, because the timing and mapping clusters net to $0.00. The journal tracer then reads each journal and its support.

Does it post entries to our general ledger?

No. The entry drafter drafts adjusting entries with their support and their effect on each tie. Nothing posts until a person approves; approved entries go on your next ledger batch.

Who approves adjusting entries?

The controller approves entries under $1,000,000, and the CFO co-approves entries of $1,000,000 or more. Preparer and approver are never the same person, and the approval is signed with the meaning of the signature. The limits and approvers are settings.

Will our top-side reserve entries keep showing up as breaks?

The cause finder still finds them, but as a manual journal with the reserve committee minutes beside it. The controller can keep one as a documented reconciling item; the tie then reads explained, not out of balance, and the carry-forward watcher ages it at each close.

What does internal audit get?

An audit pack with the summary, every tie, the reconciling items, the adjusting entries and who approved them, an evidence index linking each item to its source record, and the signatures. It is prepared for internal audit and your Model Audit Rule file, and exports to PDF.

Can we set our own tolerances, cut-offs and approval limits?

Yes. Tie tolerance, line-match window, the evidence a cluster needs, the cut-off time of each system, approval limits and approvers, the carried-item escalation, the close calendar and the cost code mapping table are all settings.

How long does it take to go live?

The Agentic Solution Engine builds and deploys it from your requirements and documents — your account map, close calendar, approval limits and a sample of past reconciliations — and it goes live once every quality gate has passed. We will walk you through it on your own extracts first.

See it on
