What is Medicaid Rebate Invoice Validation?

Checking each line of a state's quarterly CMS-R-144 invoice against the claims behind it before paying, and disputing in good faith the units the claims do not support. Medicaid Rebate Invoice Validation does that with six agents that match every line to the state's claim-level data, the HRSA Medicaid Exclusion File and plan rebate files, and draft a dispute for each suspect line for the rebate manager to decide.

The problem

Why every state invoice is a race against day 38

Fifty-one state programmes send a manufacturer a CMS-R-144 invoice every quarter: per NDC, per period and per utilization type, fee-for-service or managed care, the units reimbursed and the rebate claimed. Each one must be paid or disputed within 38 days of its postmark, and interest runs on late payments and on disputes the manufacturer loses. Rebate analysts check the lines in spreadsheets against the claim-level data the states send, state by state, while the clocks run.

The errors hide in hundreds of lines. A plan re-sends a year of encounters with the quantity's decimal shifted, so a 2-pen prescription is billed as 20 pens. A covered entity on the HRSA Medicaid Exclusion File bills fee-for-service, and the state still invoices units that already carried a 340B discount. A managed-care plan claims prescriptions already rebated under its own agreement, a state applies last quarter's Unit Rebate Amount, or a correction re-bills units already paid. A unit nobody had time to check is a unit paid.

One state invoice, from postmark to paid and disputed

Northwind Bio, labeler 71842, sells Veltrimab pens, Corventa tablets and Talmirex vials. New York's 2Q2026 invoice arrives on Oct 5 through the state portal: 24 lines, 16 of them for earlier quarters, $51,739,198 claimed. Here is what happens next, screen by screen, in the working solution.

  1. 01Wednesday, Oct 7

Every state invoice by stage and days left

Lena Ortiz · Medicaid rebate manager

Lena owns the quarter: 48 of 51 invoices are in. Five are waiting to be scrubbed, seven need her decision, four letters wait for her signature and 32 states are paid. New York is at the top of the list: 24 lines, $51.74M, postmarked Oct 1 and due Nov 8, 32 days left.

“Every line is matched to the state's claim-level data, then tested five ways. A suspect line gets a dispute drafted with its evidence; you approve it.”

  2. 02One click

“Scrub 24 lines” — the claims behind every line

    The agents

The invoice intake agent has already read the 24 CMS-R-144 lines into one format. The claim-data matcher now works down the ledger, line by line, matching the state's claim-level data on prescription number, fill date and NDC. The units and pricing checker and the duplicate-discount checker test each line as it is matched, and a suspect line turns red as soon as it is found.

    On screen: “Claim-data matcher · line 13 of 24 · 7,619 claims matched.”

  3. 03Seconds later

Fourteen lines at ten times the claims

    Units and pricing checker

Fourteen managed-care adjustment lines for 2Q2025 to 1Q2026 are red, each with the same reason: “10× the claims · decimal shifted”. Line 1 invoices 240 Veltrimab 40 mg pens on 12 prescriptions; the claims support 24, 2.0 per prescription. The strip on top reads $51,739,198 invoiced, minus $4,294,991 suspect, equals $47,444,207 supported by the claims.

“Recommended: Dispute 216 units — $387,720.00. Pay the 24 units the claims support and ask the state to re-run Kestrel's batch with the quantity corrected.”

  4. 04Line 14

    300 tablets on a 30-tablet bottle

    Claim-data matcher

Every suspect line comes from Kestrel Family Health's late encounter batch, re-sent to the state on Aug 14 after the plan moved claims systems. On the 2Q2025 Corventa line, 3,600 tablets are invoiced on 12 prescriptions — 300 per prescription for a bottle of 30 — while the plan paid the price of a normal fill. Kestrel's own history is 30.0 per prescription for eight quarters. Each statement links to its source: the claim-level data, the product listing and the units history.

Below 85% confidence, the units and pricing checker recommends a claim-data request, not a dispute.

  5. 05One click

Fourteen disputes drafted, each with its evidence

    Dispute letter writer

“Draft 14 disputes” writes one dispute per suspect line: the reason code “Units not supported by claim data”, the units disputed — 216 of 240 on line 1 — the amount, $387,720.00, and the evidence behind it. Each dispute carries its own tracker: Drafted, Approved, Sent with payment, State reviewing, Resolved.

Activity: “Dispute letter writer drafted 14 disputes — $4.29M, each with its claim evidence.”

  6. 06Approved

She approves; the letter writes itself

Lena Ortiz · Medicaid rebate manager

Lena approves the 14 disputes and asks for the letter. It is written from her decisions only: the amount paid for undisputed utilization, every disputed line with its NDC, period, units and amount, the reason in plain words, and an offer of a dispute resolution meeting. The matching reconciliation statement lines, form CMS-304, go with it.

“We are paying $47,444,207.20 for all utilization that is not in dispute, before the due date of Nov 8. Under the rebate agreement, we dispute in good faith the units below, totalling $4,294,990.80.”

  7. 07Signed

    Above $1M, counsel co-signs

    Lena Ortiz and Dana Okafor

The sign-and-send screen shows exactly what she is committing to: pay $47,444,207.20 with a value date of Nov 2, dispute $4,294,990.80 on 14 lines, the letter to New York's Medicaid drug rebate unit through the state portal. The disputes are above $1,000,000, so Dana Okafor, counsel for government programs, co-signs.

“Your signature, the letter and the payment file are recorded on the audit trail.”

  8. 08Paid · day 32 of 38

The undisputed amount paid, every dispute opened

    Payment and accrual agent

On signature the payment and accrual agent releases $47,444,207.20 for a Nov 2 value date, six days before interest would start, and posts the true-up between New York's Medicaid reserve and the validated invoice. Disputed dollars stay in the reserve. Fourteen dispute records open, from D-1236, each marked “Letter sent Oct 7”.

Activity: “Payment and accrual agent released $47,444,207.20 and posted the true-up.”

Ohio: five findings, five different checks

Lena Ortiz · Medicaid rebate manager

Ohio's $20,892,451 invoice has five lines to decide, $2,080,102 in all. Meadowlark Health Plan reported 4.0 Veltrimab pens per prescription against 2.0 for every other plan, and the state paid $1,599 a pen against $3,198 for the others — every pen counted twice. The others: covered entities on the exclusion file ($173K), Tallgrass Community Plan's claims already rebated ($433K), last quarter's Talmirex URA of $163.2000 instead of $151.4000 ($13K) and a 4Q2025 correction that re-bills tablets already paid ($296K).

“Recommended: Dispute 620 units — $1,165,600.00. Ask the state to correct Meadowlark Health Plan's units; the other 2,230 units are paid.”

  10. 10Until the state credits it

Every dispute tracked, with what it costs to lose

    Payment and accrual agent

New York's 14 disputes join the others: 58 open, $13.93M held in reserve, $170K of interest if every one were lost today. The oldest, D-1088 — Texas, 3Q2025, a 340B duplicate — is 335 days old with a resolution meeting booked for Oct 15. Each week state replies are read, disputes updated and the exposure recomputed; $3.67M has been credited by states this year.

6 agents. Each with one job, and hard limits.

Six specialist agents scrub, match, check and draft every invoice line; the rebate manager approves each dispute.

Invoice intake agent

Collects each state's CMS-R-144 invoice from state portals, secure email and SFTP every morning, and reads text, CSV or PDF into one line format: NDC, period, utilization type, URA, units, amount claimed, prescriptions and the correction flag. It starts the 38-day clock from the postmark.

  • Unreadable fields go to a person, never guessed
  • A postmark date is required to start the clock
Claim-data matcher

Matches the state's claim-level data, plan rebate claim files and the HRSA Medicaid Exclusion File to each invoice line, and checks that claim units add up to the invoice units. When claim-level data is missing, it drafts the request to the state.

  • Matches on prescription number, fill date and NDC — never on amount alone
Units and pricing checker

Compares units per prescription for each plan with its last eight quarters and with the product's package and dosing, the amount paid per unit with WAC, the state's Unit Rebate Amount with the quarterly file, and each correction with the state's notice of method and our payment history.

  • Every finding cites the claim rows or file lines behind it
  • Below 85% confidence it recommends a claim-data request, not a dispute
Duplicate-discount checker

Matches fee-for-service claims to covered entities on the HRSA Medicaid Exclusion File for the quarter, by NPI and Medicaid billing number, flags managed-care claims with 340B identifiers, and matches managed-care claims to plan rebate claim files prescription by prescription.

  • An entity counts only if it is listed for the dates of service
Dispute letter writer

Writes one state-specific letter per invoice from the decided lines: the amount paid for undisputed utilization, each disputed line with units, amount and reason, the evidence in plain words, and an offer of the state's dispute resolution meeting — with the matching CMS-304 lines.

  • Only decided lines appear in a letter
  • Counsel co-signs above the approval amount
Payment and accrual agent

On signature, creates the payment file for the undisputed amount with a value date before day 38, posts the true-up between the Medicaid reserve and the validated invoice, opens one dispute record per disputed line, and each week reads state replies and recomputes interest exposure.

  • Payment needs a signed letter or a clean invoice
  • Journal entries over $5M need accounting approval
Rebate manager

Decides every flagged line. The agents propose; a named person decides.

Ask in plain words

Ask about any invoice, finding or dispute

Ask about the 38-day clocks, units per prescription, 340B and managed-care duplicates, Unit Rebate Amounts, letters or interest — or say what to change. Every answer points to its source.

Why are New York's Kestrel lines ten times the claims?

14 of New York's 24 lines are managed-care adjustments for 2Q2025 to 1Q2026 from Kestrel Family Health's late encounter batch. On every claim the quantity is ten times what the package and days' supply hold — 20 pens for a 2-pen carton, 300 tablets for a 30-day bottle — while the plan paid the price of a normal fill. Suspect: 15,687 units — $4,294,990.80.

Which states are due this week?

Six states are due within 7 days: Texas, Illinois, North Carolina and Washington have letters to sign, due Oct 9 to Oct 11; Ohio has 5 lines to decide ($2.08M), due Oct 11; Pennsylvania has 2 ($621K), due Oct 12. Payment goes out on day 32, so these are the ones to clear first.

Why are Ohio's Veltrimab units disputed?

One plan, Meadowlark Health Plan, reported 4.0 pens per prescription on 310 prescriptions. Veltrimab 40 mg is a carton of 2 pens for a 28-day supply and every other plan averages 2.0. The plan paid the same per prescription, so the amount per pen halved — every pen was counted twice. Recommended: dispute 620 units — $1,165,600.00.

What is our interest exposure on open disputes?

If every open dispute were lost today, interest would be $169,623 at the 91-day Treasury bill rate (3.90%). The largest is D-1094, California, $2.01M, at $71,182. The oldest — Texas 3Q2025, 340B — has a resolution meeting on Oct 15.

Questions

What Medicaid rebate teams ask us.

How does it find unit-of-measure errors?

Two signals together: units per prescription against the plan's last eight quarters and the product's package, and the amount the state paid per unit against WAC — when units double for the same payment, the amount per unit halves. A line is flagged above 1.5× the usual or below 60% of WAC; both are settings.

Does it catch 340B duplicate discounts and managed-care overlap?

Yes. The duplicate-discount checker matches fee-for-service claims to covered entities on the HRSA Medicaid Exclusion File for the quarter, flags managed-care claims with 340B identifiers, and matches managed-care claims to each plan's rebate claim file prescription by prescription. An entity counts only if it is listed for the dates of service.

What if a state does not send claim-level data?

In the working solution 44 states send it routinely. For the rest, the claim-data matcher drafts the request the day the invoice lands. A line can be paid now with a request for claim-level data, and the request goes with the letter; below 85% confidence the checker recommends that instead of a dispute.

How are dispute letters and the CMS-304 written?

From the rebate manager's decisions only. The dispute letter writer states what is paid and by when, lists each disputed line with units, amount and reason, explains the evidence in plain words and offers the state's dispute resolution meeting, and builds the matching reconciliation statement lines on form CMS-304.

Do people stay in control of what is paid and disputed?

Yes. The agents read, check and draft; they never sign or pay. The rebate manager approves every dispute and signs, counsel co-signs above $1,000,000, accounting approves journal entries over $5M, and payment needs a signed letter or a clean invoice.

Does it track disputes until the state credits them?

Yes. One dispute record opens per disputed line and follows it from letter sent through state reviewing, claim data received and the resolution meeting to resolved. Each week state replies are read and interest exposure is recomputed at the 91-day Treasury bill rate.

How long does it take to go live?
