What is Loan Document Exception Tracking?

It is how a bank keeps track of the documents a loan should have and does not — unrecorded mortgages, missing signatures, expired insurance, absent organisational documents — and chases them until they are in the file. Loan Document Exception Tracking puts every exception on one board sorted by risk, sends the chases on a cadence and reads every returned document before it clears.

The problem

Why loan document exceptions sit on the report for months

Every commercial bank keeps an exception report: the documents a loan should have and does not. A recorded mortgage that never came back from the county, a guaranty with one signature, an expired hazard certificate, a missing operating agreement. Clearing one means a chase email, a follow-up, another follow-up copying the banker, and someone reading what finally comes back.

Nobody has the time, so the report grows — and a spreadsheet tickler or exception tracker only knows that a file arrived. A mortgage recorded without every owner’s signature, or a certificate naming the bank as additional insured instead of lender’s loss payee, gets marked cleared because something landed in the folder. Examiners read the same aging report, and they count the oldest items first.

Nine returned documents, from Received to cleared — or back

Monday 12 October, 08:40, at Carrick Hill Bank’s commercial loan documentation desk. Nine documents came back since Friday. Here is what happens next, screen by screen, in the working solution.

  1. 01Monday 08:40

Every exception on one board, the riskiest on top

Teresa Okafor · Commercial loan documentation manager

168 open across 1,862 commercial loans: Missing 88, Requested 71, Received 9, with 46 cleared and 7 waived in the last 30 days. Each card carries the borrower, the document, the loan number, the amount, a severity stripe, the banker’s initials and an aging bar that turns amber at 60 days and red at 90. Columns are sorted by risk, then age, so lien items rise to the top.

“9 documents came back since Friday. Nothing clears until it is read: signers, acknowledgments, names, recording, amounts.”

  2. 02One click

“Read today’s returns” — every page, every check

    Returned-document checker

Teresa presses one button. The checker works down the Received column one document at a time, and the checks tick on each card as it reads. Seven have already cleared; now it is on the Brightwater Dental Partners hazard insurance certificate from Haskett Insurance Agency — one page, six checks. A document that passes every check slides into Cleared.

Received is a checkpoint, not a pile: the only way into Cleared is through the check.

  3. 03Run finished

Seven clear with their proof. Two come back.

    Returned-document checker

Nine documents, 79 pages. Seven cleared, each with the page that proves it — a UCC-1 filing acknowledgment under the exact debtor name, a flood policy at $500,000 per building, a guaranty signed and notarized. Two bounced to Requested with the reason written on the card: the Tallgrass recorded mortgage, “Unsigned by second guarantor”, and the Brightwater Dental hazard certificate, “Bank named as additional insured, not lender’s loss payee”.

“Today’s returns read · 9 documents, 79 pages · 7 cleared with their proving page · 2 back to Requested with the reason on the card”

The mortgage, with a note pinned to every check

Teresa Okafor · Commercial loan documentation manager

Tallgrass Cold Storage LLC, loan 4410-2287, $6,850,000. The mortgage came back recorded — Marrow County, instrument 2026-008812, 2 Oct 2026 10:42 — and six of eight checks pass: recording stamp, mortgagors against the title vesting, mortgagee, amount and maturity against the note, legal description against title commitment PTA-26-11873, Owen Strand’s signature. On page 14, Mitchell Ravel’s signature line is blank, and the notary acknowledgment names Owen Strand only.

  5. 05What it means

Half the warehouse is outside the mortgage — and the fix is drafted

    Document chaser

Owen Strand and Mitchell Ravel own 2250 Depot Road half each, as tenants in common. With one signature, Mitchell Ravel’s 50% — about $4,550,000 of the $9,100,000 appraisal — is not under the mortgage, and credit approval condition 4 requires both owners as mortgagors. The chaser has drafted the corrective chase to closing counsel Hal Pruitt, cc Kara Lindqvist and Colin Mercer: a ratification and joinder for Mitchell Ravel to sign before a notary, re-recorded with Marrow County.

“First message to this contact on this exception — it waits for you.”

  6. 06Sent · replied

Teresa sends it. The signing is booked.

Hal Pruitt · Closing counsel, Pruitt & Lowe LLP

Teresa reads the draft and sends it. Hal Pruitt replies: “Thanks — that’s on us. Mitchell will sign a ratification and joinder here on Wednesday 14 October at 10:00 and we’ll re-record with Marrow County the same afternoon.” The chaser reads the reply and files it to the exception; the card now shows signing Wed 14 Oct, with the next look on Thursday 15 October.

  7. 0710:00 every business day

    Every chase on one cadence

    Document chaser

Ask on day 0, remind on day 7, copy the banker on day 14, the manager on day 21. The outbox shows each thread on its cadence ribbon: Sumner & Vale’s assignment of rents waiting to escalate after three unanswered messages to Lakeshore Title, Delaney Brothers’ guaranty handed to the banker to call at 212 days, a UCC-3 continuation drafted for Northfork Dairy Farms before the 2 Feb 2027 lapse. Escalations, banker calls and first messages wait for a person.

412 messages this month by the document chaser · 0 to an unknown contact.

  8. 08Waiver requests

A waiver ask, with a recommendation — sometimes “decline”

    Waiver drafter

Colin Mercer asks to waive the recorded copy of Cobalt Ridge Hospitality’s deed of trust, open 131 days. The lien and filing watcher has already found it in the Summit County index, recorded 21 Sep 2026 as instrument 2026-041177. The drafter recommends declining and ordering an $18 certified copy instead — and because it is a lien item, the decision goes to the chief credit officer, Alan Prescott.

Lien and recording items are not waived below the chief credit officer (policy 7.1.4).

  9. 09Every Monday, 07:00

The aging report the examiners will read

    Aging reporter

Open exceptions by banker, days open and severity, in the form that goes to the credit committee and the examiners. Over twelve weeks the board fell from 263 open to 168, and from 74 over 90 days to 31. Critical items are 8% of the count and 41% of the exposure: $78.4M of loans with a lien or recording item open.

  10. 10Any loan

The closing checklist, matched to the file

Colin Mercer · Relationship manager, commercial real estate

Tallgrass’s 22-item closing checklist and approval conditions: what is in the file, what is an exception and where each one sits on the board. The assignment of leases and rents shows cleared today; the mortgage shows back to Requested, unsigned by the second guarantor; the final title policy is still missing — it issues after the mortgage is recorded.

Six agents chase, read and check every missing loan document; the documentation manager approves waivers.

Exception intake

Turns the loan system’s exception report, the closing checklist and the approval conditions into one card per missing or defective document, with its severity and owner, merges duplicates and flags items policy does not require.

  • Never closes or waives an exception
  • Severity comes from Settings, not judgment
Document chaser

Writes each chase to the right party — borrower, title company, closing counsel, insurance agent or banker — on the day 0 · 7 · 14 · 21 cadence, reads the replies and files what comes back to its exception.

  • First message to a new contact waits for a person
  • Never writes to a contact who is not on the loan
  • Never mentions a waiver to a borrower
Returned-document checker

Reads every returned document against the loan before it can clear: signers, notary acknowledgments, dates, exact legal names, recording stamp, legal description, amounts and insurance wording.

  • Never clears on a partial pass
  • Cites the page for every check
  • Never edits a document
Lien and filing watcher

Watches UCC-1 lapse dates, county recording indexes and hazard and flood expiries, and opens an exception before a lapse becomes a loss.

  • Never files with a state or county
  • Continuations only inside the six-month window
Waiver drafter

Turns a banker’s ask into a waiver request with the policy, the collateral at stake, compensating factors and a recommendation: approve, approve with a condition, decline, or close as not required.

  • Never approves — a person decides
  • Critical always goes to the chief credit officer
Aging reporter

Builds the Monday aging report by banker, severity and days open for the credit committee and the examiners.

  • Never re-dates or hides an exception
Documentation manager

Ask in plain words

Ask about any exception, in plain words

Ask about any exception, loan, banker or chase — or tell it who to chase. Every answer points to the page or record it rests on.

Why did the Tallgrass mortgage go back to Requested?

The recorded mortgage came back from Marrow County as instrument 2026-008812, but page 14 is signed only by Owen Strand; Mitchell Ravel’s signature line is blank and the notary acknowledgment names one signer. The two own 2250 Depot Road as tenants in common, so Mitchell Ravel’s 50% — about $4,550,000 of the $9,100,000 appraisal — is not under the mortgage. A ratification and joinder to Hal Pruitt is drafted.

Which liens are not perfected right now?

Fourteen Critical items are open. The most exposure sits with Cobalt Ridge Hospitality (deed of trust recorded on 21 Sep, copy not in file), Sumner & Vale Logistics (assignment of rents not recorded), Northfork Dairy Farms (UCC-1 lapses 2 Feb 2027 — continuation window open), Tallgrass Cold Storage (mortgage missing Mitchell Ravel) and Bellhaven Medical Plaza (mortgage at the county, back 16 Oct). Only Sumner & Vale and Tallgrass are truly unperfected today; Northfork is if nobody files by 2 Feb.

Show Colin Mercer’s exceptions over 90 days

Colin Mercer has 41 open, 10 of them over 90 days — the most of any banker. The oldest is Cobalt Ridge Hospitality’s deed of trust at 131 days; he has asked to waive it, but the county index shows it recorded on 21 Sep, so a certified copy clears it instead. Seven of his ten are title and recording items from two title companies.

Ask Prairieland Title for everything they owe us in one message

Drafted one message to Dee Halvorsen at Prairieland Title & Abstract covering three items on two loans: the Bellhaven Medical Plaza recorded mortgage, promised 16 Oct; the Tallgrass final title policy, after the joinder is re-recorded; and the Tallgrass date-down after the re-recording. It replaces the two separate reminders in the 10:00 batch and is waiting for you on Chases.

Governance

Built for loan documentation: cited, checked, approved, on the record.

Every clearance cites its pageA cleared exception carries the page that proves it; a bounced one carries the page that fails, with the defect written in one line on the card.
Nothing clears on a partial passA returned document clears only when every applicable check passes — signers, acknowledgment, recording, names, legal description, amounts, insurance wording. Six of eight is a bounce.
A named person approves every waiverThe documentation manager approves waivers from Low to High; a lien or recording exception is waived only by the chief credit officer. The agents recommend; they never approve.
Clearing without the document takes two peopleClearing an exception without its proof needs a written reason and a second approver, and it shows on the aging report as an override.
Chases go only to people on the loanThe chaser never writes to a contact outside the loan and never promises a waiver. The first message to a new contact, escalations and banker calls wait for a person.
Every step on the recordEvery chase, reply, check and decision is on the exception’s activity trail. Settings changes are versioned and on the audit trail, and no exception is ever re-dated or hidden from the aging report.

Questions

What documentation teams ask us.

How is it different from a tickler or exception tracking spreadsheet?

A tickler tracks that a document is due and that a file arrived. Here, Received is a checkpoint: the returned-document checker reads every page against the loan — every required signer, the notary acknowledgment, the recording stamp, the legal description, amounts and loss payee wording — and only a document that passes every applicable check clears.

What happens when a returned document is wrong?

It goes back to Requested with the defect written on the card and the page that fails it. The document chaser drafts the corrective chase — for the Tallgrass mortgage, a ratification and joinder to closing counsel — and it waits for the documentation manager to send it.

Does it file UCC continuations or record documents?

No. The lien and filing watcher watches UCC-1 lapse dates, county recording indexes and insurance expiries, and opens an exception when the six-month continuation window opens. It drafts what is needed; it never files with a state or county.

Do people stay in control?

Yes. Agents recommend; people decide. The documentation manager approves waivers from Low to High, the chief credit officer approves any lien or recording waiver, clearing without the document needs a reason and a second person, and the first message to a new contact waits for a person.

Can we set our own severity rules and chase cadence?

Yes. Severity by document type, the reminder day, when the banker is copied and when the manager is escalated, the send window, the freshness of good-standing certificates, the aging thresholds and who approves waivers are all settings, versioned and on the audit trail.

Which systems does it work with?

Your loan system’s exception report, closing checklists and credit approvals, bank email for chases and replies, county recorder indexes and Secretary of State UCC search, an insurance tracking feed and your commercial loan policy.

How long does it take to go live?

The Agentic Solution Engine builds and deploys it from your requirements and documents — your loan policy, exception report, closing checklists and contacts — and it goes live once every quality gate has passed. We will walk you through it on a sample of your own open exceptions first.
